A well-maintained LOTO register ensures compliance to local safety standards and most importantly, avoids workplace injury due to sudden energy release. The following are the elements of a good LOTO register:

1. Equipment Details

The lockout tagout register will include details of each piece of machinery or equipment that must be isolated, which includes a unique Equipment ID/name, its location, and the type of hazardous energy involved (e.g., electrical, hydraulic, pneumatic, or mechanical).

2. Energy Isolation Points

Document all devices of isolation such as circuit breakers, valves, or switches) and the means of isolation like lockout, tagout, or physical removal of parts. Doing so provides a definitive description of how energy sources are secured.

3. Authorised Personnel

Only trained and authorised personnel should conduct LOTO operations to avoid hazards, and the register should contain their names, signatures, and training dates to ensure competency and adherence to workplace safety practices.

4. LOTO Activity Log

Every lockout/tagout event must be recorded with date, time, and cause like maintenance, repair, or inspection. Lock and tag numbers must be recorded at all times by authorised people for tracking and preventing unauthorised removal.

5. Testing and Verification

Prior to initiating work, there will be a verification process to ensure zero energy, such as residual pressure testing, or test starts, and in suitable cases, a witness signature will be obtained to confirm isolation.
